Why Ovulation Tests Give False Positives With PCOS

If your ovulation tests keep reading positive and nothing seems to follow, you are not using them wrong. There is a specific reason this happens with PCOS.

What the test is actually measuring

An ovulation predictor kit does not detect ovulation. It detects luteinising hormone — LH — in your urine.

In a typical cycle, LH sits low, then spikes sharply for roughly 24 to 36 hours. That surge is what triggers the ovary to release an egg. The strip is calibrated to that pattern: low background, sudden spike, so a positive line means the spike is happening and ovulation is likely to follow.

The whole design depends on the background being low.

Why PCOS breaks the assumption

In PCOS, baseline LH is frequently elevated — not surging, just persistently higher than the test expects. It is a well-recognised feature, and it is why doctors sometimes look at LH relative to FSH when assessing PCOS.

Put that chronically raised LH into a test designed to spot a spike above a low baseline, and you get exactly what so many people describe: positive after positive, across days or even weeks, with no evidence that ovulation ever occurred.

The test is not faulty and neither are you. It is answering the question it was built for, and that question assumes a hormone pattern PCOS often does not follow.

The second problem: false starts

There is a related pattern that is not a test error at all. With PCOS, the body may make several genuine attempts at ovulation in one cycle — LH rises, a follicle begins to mature, and then it stalls without releasing an egg.

Each of those attempts can produce a real LH rise and a real positive test. So you can get a true positive that is nonetheless not followed by ovulation. Two or three of those in a single long cycle is common, and it is one reason a 50-day cycle can contain several hopeful weeks.
